Stage 1: Early Signs and Subtle Symptoms

At this stage, things may seem minor—but they’re worth paying attention to.

You might notice:

  • Mild leakage with sneezing, coughing, or running

  • Occasional pelvic pressure or heaviness

  • Difficulty fully emptying your bladder or bowels

  • Low back or hip tightness that won’t go away

  • Discomfort during or after intimacy

Stage 2: Persistent or Worsening Symptoms

In this stage, the symptoms have become more frequent or disruptive. You might notice:

  • Regular incontinence (leakage during exercise or daily activities)

  • Ongoing pelvic pain or pressure

  • Pain with penetration or tampon use

  • Difficulty controlling gas or stool

  • Constipation that doesn’t resolve with hydration or diet changes

  • Chronic low back, SI joint, or hip pain

  • Postural changes due to guarding or discomfort

  • Pelvic organ prolapse symptoms (a sensation of bulging or heaviness)

  • Emotional or mental strain from dealing with ongoing dysfunction

At this stage, the issue is rarely isolated. The pelvic floor is connected to the entire core and movement system, and a comprehensive treatment plan is essential.

You may need:

  • Pelvic floor retraining

  • Breathwork and nervous system regulation

  • Hands-on manual therapy

  • Functional strengthening and mobility work


Stage 4: Post-Surgical or Complex Cases

Some individuals may reach a stage where surgical intervention (like for prolapse or endometriosis) becomes part of the picture. But even here, pelvic floor therapy plays a vital role in recovery and long-term function.
